Webinar overview
The aged care landscape is changing, and more older Australians are choosing to be cared for in their final days at home. Recent reforms and new funding initiatives will require providers of community aged care to have end-of-life care embedded into their service provision including supporting individuals to be cared for and to die at home if that is their wish.
This webinar will use a case study to introduce a clinical framework to guide proactive care planning and ensure timely symptom management for individuals with life-limiting disease. Resources will be provided to support health professionals in delivering personalised, evidence-based care empowering clients to make informed decisions in their last 12 months of life and supporting their families.
Who should attend?
This training is ideal for registered nurses, case managers and clinical managers providing aged care in the community.
The outcomes are:
- use a framework to proactively plan, with the involvement of clients and their families for a death at home
- utilise policies, guidelines and checklists to align with quality standards and best practice
- liaise and collaborate with members of interdisciplinary and interagency teams to ensure best outcomes for clients and families
- utilise caring@home resources to empower families and carers to manage symptoms and practical cares at home

Who is facilitating?

Jane Stephens
Jane Stephens is the Clinical Nurse Consultant for the caring@home project – a national project that aims to increase access to quality and timely end-of-life care for patients who choose to be cared for and die at home. Her role is to provide clinical expertise to the development and provision of nationally consistent, practical, and evidence-based clinical resources and education for health professionals and to improve experiences for patients and their families in the community.
Jane’s work in the field of palliative care spans over 20 years in clinical, research, project and community settings. Her major interest is ensuring that people who wish to remain at home at end-of-life receive quality care and that their families are well supported.
caring@home is funded by the Australian Government and led by the Brisbane South Palliative Care Collaborative.

Cancellations, refunds, substitutions and transfers
Ageing Australia’s learning and professional development (L&PD) products are delivered subject to sufficient registrations, as stipulated by available tickets against each event, and tickets can be purchased directly from the what’s on page.
Training session viability of each advertised session is assessed one week prior to the scheduled date of delivery, when a decision is made regarding continuation or cancellation. When a training session is cancelled due to insufficient numbers or circumstances outside of the control of Ageing Australia, a full refund will be paid, directly to the customer who has made the initial purchase.
A full refund is also available if a participant withdraws from a training session, five or more days prior to the session being delivered, on the condition, that the customer has contacted , to inform regarding their intention to withdraw. No refunds or transfers will be considered without notification prior to the commencement of a training session. Late notifications will be considered on a case-by-case basis.
An organisation may choose to substitute a participant of a previously paid session, by emailing , 24 hours prior to the commencement of the training session. Full details of the substituted participant must be provided.
Payment terms
It is a condition of registration that participants pay full payment at the time of booking via credit card or by invoicing by prior arrangement. All payments must be finalised prior to commencement. Registrants who have outstanding payments will not be able to participate. Upon registration, a tax invoice will be emailed to the person making the booking.
